Keep an incident register that supports correction
Record business effect, severity, evidence, ownership and resolution without erasing open or accepted risk.
Record occurrence
- Use stable event IDs and actual occurrence dates.
- Describe the affected business result, not only the technical symptom.
Preserve state
- Distinguish open, monitoring, resolved and accepted risk.
- Link evidence and the accountable owner.
Close carefully
- Record resolution and downstream reconciliation.
- Do not call a retry a resolution until its effect is known.
Keep the evidence connected
The Operations Assurance Studio retains workflow identity, delivery context, operating observations and the next decision. It does not certify uptime, prove causation or authorize an external action.
